LPN or RN – Remote Patient Monitoring (RPM) Program
(In clinic position, not a remote position)

Job Duties:
- Assist Provider and QSS Care Team to identify patients qualifying for the RPM program.
- Connect with patient and deploy remote monitoring equipment.
- Perform initial intake and provide patient training on the equipment.
- Monitor patient progress under direction of the patient’s Provider.
- Review and analyze data to improve patient care.
- Provide medical and wellness education under the direction of patient’s Provider.
- Remotely triage and encourage patient participation in their care via video, phone or in person.
- Responsible for completeness and accuracy of patient information entered into the EMR.
- Effectively manage preventative maintenance needs on equipment to minimize downtime.
- Ensure patient monitoring results are proactively communicated to the Provider for complete patient centered medical home health care.
Qualifications/Skills:
- Current RN or LPN license in good standing with the State of Louisiana.
- Current Basic Life Support (BLS).
- Experience in ambulatory care setting, preferred.
Employment Type: Full Time, In person position Monday through Friday.

Starting Salary Range:
- LPN - $18.66 – $24.35 per hour; up to 3% annual increases.
- RN - $50,230 - $71,240 per year; up to 3% annual increases.
