Job Duties:
- Triage Coordinate with providers and nurses to resolve health concerns.
- Focus on assessments.
- Integrated Care Promote the delivery of care across the continuum of services.
- Care Gap attainment of identified preventive and chronic disease gaps in care.
- Failed Appointments Follow-up on cancelled/no show appointments.
- Lab tracking/Medical - Monitor lab orders.
- Data - Prepare data on Referrals.
Qualifications/Skills:
- Current RN or LPN license in good standing with the State of Louisiana (Required).
- Current Basic Life Support (BLS) (Required).
- Prefer experience in ambulatory care setting.
Employment Type: Full Time, Monday through Friday

Starting Salary Range:
- LPN - $18.66 – $24.35 per hour; up to 3% annual increases.
- RN - $50,230 - $71,240 per year; up to 3% annual increases.
